The Work Request Form allows your customers to request services for their vehicle straight from your website or a direct link. The information they submit will carry over to a newly created estimate in Shopmonkey. You can customize the entry fields on your Work Request Form, giving you control over what information you receive from your customers.

Customize Your Work Request Form

Select the Settings icon in the bottom left corner of your screen.

Select Work Request Form under the Settings section on the left-hand side.

The Work Request Form page will show you a preview of what the form will look like on your website after you embed the form. To make changes to the Work Request Form, select Edit.

In the Edit Work Request Form Menu, change the appearance and information you require or request from customers.

Make changes to the form's contents including Form Format, Header, Personal Info, Vehicle Info, Additional Info, and Workflow Settings.

Form

Make changes to the format of the form.

• Form Title

• Background Color

• Button Color

Header

Toggle each on or off to include or remove shop information from the form.

• Shop Name

• Logo

• Website

• Email

• Phone

• Address

• Certifications

Personal Info

Choose the personal information you request from the customer.

• First Name (always required)

• Last Name (always required)

• Email

• Phone (When this is made required or optional for the customer, an SMS opt-in checkbox is provided for the customer on the form)

• Referral Source

• Address

You can make the customer’s response Required, Optional, or Hidden.

• Required: The customer must submit this information in order to submit the Work Request

• Optional: The customer can choose whether or not to input this information before submitting the Work Request

• Hidden: The customer will not see this field on the Work Request Form

Vehicle Info

Request Vehicle information. The customer can add this by searching by License Plate, VIN, or Year/Make/Model, or they can enter their vehicle information manually.

You can make the customer’s response Required, Optional, or Hidden.

Additional Info

Add a caption (e.g. Tell us what work you might need done on your vehicle). Make the customer’s response Required, Optional, or Hidden on the form.

After making changes to the form, select Save.

Workflow Settings

Select which workflow column new appointments (scheduled through the work request form) will be added to.

Confirmation Message

Toggle on to automatically send a confirmation message to customers after they submit the work request form. Select the message template to make changes to the Work Request Reply.

Share Your Work Request Form

Copy the URL or embed code to share the Work Request Form with your customers.

Full Page Form

Copy this URL for a full-page Work Request Form. Think of this as a separate webpage to share directly with customers. This could be something you share on social media.

Embedded Form Code Snippet

Copy this code to embed your Work Request Form on an existing page on your website.

Google Tag Manager

Select Connect to manage marketing tags and collect analytics on your forms using Google Tag Manager.
